D4 Medicals to get you back on the road
We complete medicals for group 2 license holders. These include lorry drivers, bus drivers, coach drivers, ambulance drivers, and those who wish to drive a motorhome. The form is completed by a doctor and then returned to the applicant. This is then either then returned to their employer or sent directly to the DVLA. Please come with the form that you would like completed. These will be sent be the DVLA if this is a renewal, or you can pick them up at your place of work, via the DVLA website or through the Post Office.

About the medical
Please bring your passport or driving license as a form of ID. You will also be required to bring any medication you are currently taking, your optical prescription, as well as your driving glasses. Please be prepared to take contact lenses out on the day of the assessment. The assessment will involve:
- A medical history check
- Eyesight check
- Blood pressure check
The form is completed by the doctor and the DVLA make the final decision after the assessment has been reviewed.
